The purpose of this form is to advise Sono School Of Music of your intention to take a leave of absence for a period of 2 or more consecutive weeks. (For individual appointments, please use the ONLINE SCHEDULER)
In accordance with your agreement with us, if this form is received, Sono School Of Music will:
a) delete scheduled fee payments equalling the number of weeks of leave taken such that make-up lessons do not accrue; and
b) hold your weekly appointment reservation where the leave period is not greater than 4 weeks.
c) If you take leave, we guarantee we will hold your weekly time slot for a maximum of 4 weeks per occasion. If you cannot attend for more than 4 weeks, but would like to guarantee your slot, you have the option to pay fees for the weeks in excess of 4 and cancel the appointments you cannot attend so they will stand as make-up lessons.
d) Leave cannot exceed 10 weeks over a 12 month period. If it does, we cannot guarantee your weekly time slot will continue to be reserved for you.
e) You cannot attend appointments (including using make-up lessons), while on leave.
f) Individual lessons that cannot be attended must be rescheduled; ie. leave cannot be taken for single or non-consecutive weeks.
